If communication is the problem, why does the same conversation keep coming back?

Three leadership teams. Three different businesses. Same diagnosis.
Communication.
The word was not wrong.
It was just too easy.
In each case, communication had already been worked on. Meetings had been added. Expectations had been clarified. People had agreed to use better language. Someone had probably said, more than once, that everyone needed to get aligned.
And still, the same conversation kept coming back.
That is where the case got interesting.
Because if the same conversation keeps returning after the communication work, I do not want to start by improving the conversation.
I want to know what keeps making the conversation necessary.
What pressure does it release?
What decision does it postpone?
What conflict does it make more acceptable to name indirectly?
What structure keeps producing confusion, no matter how carefully people speak?
Sometimes communication is the problem.
Sometimes it is where the system leaks.
I would keep looking underneath the words.
